Panel management is a process of proactively managing a defined population of patients, using EMR data to identify and respond to patients’ chronic and preventative care needs. To help doctors and their teams implement and sustain panel management, the GPSC has developed the Phases of Panel Management – a framework that helps doctors and their teams introduce and implement new tools and clinic processes to enable planned proactive care.

The GPSC is developing a new Panel Development Incentive to compensate doctors for the time and effort required to improve the accuracy and quality of patient data. This incentive will enhance supports for panel management, which include in-practice coaching and EMR-based panel management tools.
Improved panel management enables family doctors to understand both the patients they are taking care of, and what those patients need. The concept is simple: better information about patients leads to better care for patients. Accurate and optimized patient data is foundational to the transition to the patient medical home.
In response to feedback from physicians, PSP has moved its tools, resources, and core content to a new online platform, the PSP Exchange. On this platform, information is more streamlined and aligned to support physicians in practices with quality improvement activities for clinical care and practice management.

To comply with new College of Family Physicians of Canada (CFPC) requirements for Mainpro+ certification, the PSP has reviewed and revised content for its nine learning modules. The content was reviewed extensively by a representative group of PSP regional team members, then further reviewed by the PSP Planning Scientific Committee, a group of family doctors from across BC.
BC family doctors are working to improve services for their patients with the patient medical home (PMH) model of care. As part of their PMH journeys, more than 400 doctors have improved the quality of their patient data using PSP’s new set of EMR-based tools. The Understanding Your Patient Panel tools support doctors in managing their patient panels so that they can use the data to inform planned, proactive care.

This year, more than 160 doctors across BC have met to learn about quality improvement principles that help them find better ways to care for their patients.
