BC family doctors are working to improve services for their patients with the patient medical home (PMH) model of care. As part of their PMH journeys, more than 400 doctors have improved the quality of their patient data using PSP’s new set of EMR-based tools. The Understanding Your Patient Panel tools support doctors in managing their patient panels so that they can use the data to inform planned, proactive care.  So far, doctors who are using the assessment have:

This year, more than 160 doctors across BC have met to learn about quality improvement principles that help them find better ways to care for their patients. “Using strategies borrowed from business and industry, [doctors] are learning to identify specific practice goals, design innovative tests of change, measure and evaluate the changes, and incrementally improve their daily practice,” says Dr Davidicus Wong, a Burnaby-based family physician, in his article about how business strategies can improve health care.

On June 19 and 20, family doctors and various Ministry of Health, health authority, and Doctors of BC representatives gathered for the GPSC Spring Summit, which focused on progress and future steps toward establishing the patient medical home in BC. Keynote speaker Dr Paul Grundy, IBM’s Chief Medical Officer and Global Director for Health Care Transformation, kicked off the event, sharing his perspectives on the international emergence of patient-centred medical homes as the foundation for primary care transformation.
